Under UK law, you have a right to request access to information held about you.
You have the right to access any personal information written in your physical medical records, or stored on computer systems. If you need to see your information, please, put your request in writing to the Practice Manager or download a request form. We require to verify the identify of any patient requesting access to their records and to make the patient aware that the practice has a maximum of 30 days to complete any requests from the date the form was received. The form is available at the bottom of this section, under “Attachments”.
These requests are time consuming for the practice, and we would respectfully ask that you only make this request if there is a specific need.
